How To Refer To UVS
To ensure that all clients we see have been referred by you, their family veterinarian, we would like to receive some information from you about the client prior to us making an appointment for them.
Our phone lines are open from 8am to 5.30pm, Mon – Fri. When you call to make a referral, we will get some basic information about your client and their pet’s medical condition. We will contact your client to let them know that we have received their information and in that way they can call at their convenience to set up an appointment. We will also ask you to fax all pertinent medical history to us. This will help us avoid duplicating any testing you have already performed. In order to make the referral process as easy as possible, we have provided an online referral form which you can fill out and attach any files and file types you wish.
Using this app gives you the freedom and flexibility to make a referral directly from your phone. For more information and links to itunes and google play click here.
If you have a case which you have determined needs to be seen before our next regular appointment slot, you may request an emergency referral. In this case, please be sure to alert our front desk to the urgent nature of the case and indicate that on the referral form. It is critical that we receive records on the patient as soon as possible. We will have our Emergency Coordinator speak with you to coordinate getting your patient to us as soon as possible. Since we are not designed to be an emergency clinic, please do not send a patient to us without first speaking with our staff. This will make the experience as positive as possible for your client and their pet.
